Minimum (Up to 5 employees.)  $330.  Plus Each additional employee over  $10.

Financial Institutions      

$28. per each million of deposits.

(Minimum - $330.)  

Professionals        

(Accountants, attorneys, dentists, physicians, veterinarians etc.)

First partner   $330.

Each additional partner  $100.
