Community Food for Jobs Program – Care Coordinator  

General Description:

Under the direct supervision of the Family Development Team Leader, this Care Coordinator position is responsible for the provision of outreach intensive case management services to the Community Food for Jobs Program (CFJP) targeted population.  Services include but are not limited to:  person centered, strength based intake/assessment, development of Individual Services/Employment Plans, information and referral for needed services, service coordination with other service providers, referral follow-up, and the provision of appropriate supportive services and follow-up to foster success.  This priority will be accomplished through, but not limited to the following activities.

 

Job Duties: 

Daily/Regular:

 

  1. Provide comprehensive outreach case management services to all enrolled CFJP participants.  Major care coordination activities include, but are not limited to the following:
  2. Maintain an incoming referral and intake process to enroll eligible individuals.
  3. Conduct a person centered, strength based assessment to identify educational/employment interests/needs, skills and areas for further development;  include an assessment of support service needs as well.
  4. Within the assessment process identify literacy needs and refer for literacy tutoring as necessary.
  5. Together with the program participant, develop and Individual Service/Employment Plan establishing goals and action steps necessary to achieve desired outcomes.
  6. Determine the extent to which each participant is interested in Food Service employment and/or a Food Service Career path.
  7. Identify existing services and resources, and make appropriate referrals to such services as needed.  (The referral process may also involve transportation and accompanying the participant on the initial service visit, etc.)
  8. Coordinate Individual Service/Employment Plan activities with the CFJP Team.
  9. Provide ongoing contact and follow-up with each participant to ensure that services are beneficial and that the goal oriented action plan continues to be updated and modified as necessary; at least bi-weekly contact is required or more often as needed.
  10. Coordinate services with other involved service providers including case conferences when needed. 
  11. Maintain an individualized comprehensive record and progress report for each participant served, utilizing designed computer software.
  12. Assist in facilitating career readiness skills group training.
  13. Identify/reduce barriers and offer incentives, to ensure that program participants are able to take positive steps in accomplishing set goals.
  14. Develop linkages with EOP programs and community agencies to establish an appropriate service network path for program participants to access supportive services as necessary.
  15. Refer participants as appropriate to the EOP Job Developer and/or Food Service
  16. Career training. 

Periodic:

  1. Prepare monthly reports accounting for activities, program outputs and program outcomes.
  2. Prepare all materials necessary for effective delivery of care coordination services.
  3. Initiate and participate in case conferences as necessary to coordinate services and assure access to services.
  4. Participate in all Team and Family Development Division staff meetings.
  5. Participate in developing the annual outcome evaluation report based on performance measures and benchmarks.
  6. Provide information and technical assistance to other Agency components as necessary.

On-going:

  1. Participate in appropriate training/education programs.
  2. Participate in carrying out Agency mission and goals.
  3. Participate in appropriate task forces and other human service organizations.
  4. Update personnel file.
  5. Become an integral part of the total Agency by being aware of services provided, serving on internal work groups, attending appropriate meetings and participating in in-service training.
  6. Be available to assist in other service areas as needed.

Minimum Qualifications:

A bachelor’s degree in human service related area with at least two years experience in care coordination related activity.  An equivalent combination of college and  relevant work experience will be considered.  Evidence of person centered, strength based assessment and goal plan development is essential.  A working knowledge of community resources, career information and a positive conceptual framework for understanding the needs and interests of the population served are essential.  Evidence of professional competence in cultural diversity, communication skills, and an understanding of the cycle of poverty with all of its negative affects is a MUST.  Must have own vehicle and current driver’s license in good standing.
